Patient Information

Please make sure to fill out our paperwork prior to your visit or come early to your appointment to fill out all of the necessary paperwork.

What to expect for your appointment:

  • Please plan on being at our office for 1-2 hours.

​

  • Please bring your prescription glasses, insurance card, current photo identification, list of medications, contact info for your primary care and referring physician, emergency contact info, and any records related to your condition.

​

  • You will first meet with a technician who will assess your visual acuity, test eye pressure, ask about your medical history, instill dilation drops and perform any necessary baseline imaging.

​

  • After meeting with the technician, we will wait for your eyes to fully dilate, which can take up to 30 minutes. When your eyes are fully dilated, you will meet with Dr. Geeta Lalwani for an exam, undergo any additional testing required, and discuss findings with your Dr. Lalwani.

  • Your eyes will be dilated prior to every exam to provide a full view of the inside of your eye. For several hours following dilation, your vision may be blurred and sensitive to light. Driving may be difficult immediately following an examination, so you may consider arranging for someone to drive you home.
